Referrals

A clearer, more confident referral pathway for participants, families, and professionals.

This page is designed to help referrers quickly understand who can refer, what information is helpful, what happens next, and how supports begin.

How referrals work

1

Send an enquiry or referral

Participants, families, support coordinators, hospitals, schools, and health professionals can contact our team to start the intake process.

2

Talk through goals and support needs

We discuss current supports, priorities, funding, preferences, risks, and the kind of team or schedule that may be the best fit.

3

Match supports and confirm availability

Our team identifies the most suitable services, location coverage, and support workers based on needs, compatibility, and capacity.

4

Set up service agreement and commence

Once supports are agreed, we confirm the schedule, communication preferences, responsibilities, and begin services with a clear plan.

Who can refer?

Service agreements

We use a written service agreement so participants and providers are clear on what supports are being delivered, how they will be delivered, and how changes are managed.

Clear agreements help set expectations around schedules, communication, cancellations, responsibilities, and service reviews.
